We spend a lot of time user testing forms here at cxpartners which led us to putting together a cheat sheet for form designers.
We're pleased to share that knowledge today in the form of a free template.
We've got the Omnigraffle original, PSD (Photoshop) and PDF examples for you to download and use how you wish. Print it out, stick it on your wall, send it to your clients, generally help make everybody's forms better.
